PATH PROGRAM
Promoting Adolescent Transitions in Healthcare
WHAT TO CONSIDER
Pursuing guardianship for your child requires many considerations. We list here some important considerations for your family.
INDEPENDENCE
The extent to which the youth hopes to self-advocate and make healthcare decisions
SUPPORT
The extent to which the youth needs family support in day-to-day functioning
RESOURCES
Amount of resources currently available and whether pursuing guardianship would enable further social support
PROCESS
The process for seeking guardianship and conservatorship is a long one, and involves working with both your healthcare providers and the legal system.
The providers must provide a medical evaluation or a certificate, depending whether your child has intellectual disability.

There is also a time constraint (6 months) between petitioning the court and receiving psychological testing.
